- Pricing & Planning
One of the first questions families ask when a parent needs help at home is simple: how much will this cost? The honest answer is that it depends on the type of care, how many hours you need and where you live. This guide gives you realistic 2026 price ranges for elderly home care in Malaysia so you can plan with confidence.
Typical home care rates in Malaysia (2026)
The ranges below reflect what families commonly pay in the Klang Valley, Penang and Johor Bahru. Rates in smaller towns are often slightly lower.
- Companionship and medical escort: RM 18 – RM 28 per hour
- Personal care (bathing, toileting, feeding): RM 20 – RM 30 per hour
- Dementia care: RM 22 – RM 32 per hour
- Registered nurse home visit: RM 45 – RM 70 per hour, or from RM 90 per procedure
- Home physiotherapy: RM 130 – RM 200 per session
- Overnight caregiver (12 hours): RM 160 – RM 240 per shift
- 24-hour live-in caregiver: RM 220 – RM 350 per day
What a real monthly budget looks like
Most families do not need 24-hour care. A common arrangement for a parent who is mostly independent but needs help with bathing and meals is a caregiver for 4 hours a day, 5 days a week. At RM 25 per hour, that is RM 500 a week, or roughly RM 2,150 a month before platform or agency fees.
For a parent recovering from a stroke, families often combine a weekday caregiver with two physiotherapy sessions a week. That might come to RM 3,000 – RM 3,800 a month during the first three months of recovery, tapering off as your parent regains independence.
The cheapest quote is not always the lowest cost. A caregiver who is late, untrained or keeps changing can cost you far more in missed work days and stress.
The hidden costs to watch out for
Many families are surprised by charges that only appear after they have committed. Before you book with any agency or platform, ask about:
- Registration or matching fees charged upfront, often RM 300 – RM 1,000
- Refundable deposits that are hard to get back
- Lock-in contracts with minimum months
- Unclear commission: you pay RM 30 an hour but the caregiver receives RM 15, which leads to high turnover
- Public holiday, overtime and transport surcharges that are not disclosed upfront
How transparent pricing works on Love Care
We built Love Care because families and caregivers both deserve to know where the money goes. Each caregiver sets their own rate. The family pays that rate plus a 10% platform fee, and the caregiver receives their rate minus a 10% platform fee. Both sides see the same breakdown on every booking.
For example, if a caregiver's rate is RM 25 per hour for a 4-hour visit (RM 100), the family pays RM 110 and the caregiver receives RM 90. The RM 20 platform fee covers verification, matching, payments, insurance coordination and our live support team. No deposits, no registration fees and no lock-in contracts.
Can I get financial help?
Some families use EPF (KWSP) savings of the parent, private medical or long-term care insurance, or support schemes from welfare bodies and NGOs. Check your parent's insurance policy for home nursing or post-hospital care benefits, as some plans reimburse care after a hospital stay.
The bottom line
Home care in Malaysia can be surprisingly affordable when you only pay for the hours you need and the pricing is open. Start with a clear list of your parent's needs, ask every provider for a full breakdown, and choose the option where you understand exactly what you are paying for.
